Freshmen Requirements :

  • Form 138 (Original High School Card)
  • Certificate of Good Moral Character
  • Birth Certificate (Authenticated from NSO)
  • (2 pcs)2x2 ID picture (2 copies-recent)
  • (1 pc) 1x1 ID picture with plain background for the library.
  • Passed successfully Eye and Hearing Examinations. (at UC-METC clinic)
  • Php 500.00 as Entrance Payment
  • Payment for Mailing of Grades/Documents (UC-METC Cashier)

Old Students Requirements :

  • Non-Graduating
    • Print out of Midterm Grades, cut off grade is 2.5 (Below 2.5 needs confirmation from individual instructor)
    • Semestral Clearance
    • P500.00 as Entrance Payment
    • Payment for Mailing of Grades/Documents (UC-METC Cashier)
  • Graduating
    • Evaluating of Subjects taken and passed.
    • Print out of Midterm Grades of the first semester/second sem
    • Semestral Clearance
    • P500.00
    • Payment for Mailing of Grades/Documents (UC-METC Cashier)

Students who incur two(2) failing grades in Professionl subjects including English in a given semester will be disqualified from pursuing the BSMT/BSMAR-E programs except for meritorious cases, effective S.Y. 2000-2001.

Reasons for failure (3) other than academics are governed by the rules and regulations of CHED, other concerned agencies, and the School administration

Shiftee / Returnee Requirements :

  • Periodic Grade Report (Final Grade of the last Attendance)
  • Shiftees/Returnees Clearance Form (For verification of school credentials and accounts).
  • 1 x 1 ID picture with plain background - 1 copy for the Library.
  • P500.00 as Entrance Payment
  • Payment for Mailing of Grades/Documents (UC-METC Cashier)
  • Passed successfully Eye and Hearing Examinations (at UC-METC Clinic)

Transferee Requirements :

  • Certificate of Transfer Credentials
  • Informative Copy - Transcript of Records (with good scholastic records)
  • Certificate of Good Moral Character
  • Birth Certificate (Authenticated from NSO)
  • 2 x 2 ID picture (2 copies - recent) and 1 copy 1 x 1 ID picture with plain background for the library.
  • P500.00 as Entrance Payment
  • Payment for Mailing of Grades/Documents (UC-METC Cashier)
  • Passed successfully Eye and Hearing Examinations (at UC-METC Clinic)
Foreign Student Requirements :
  • Fill in all the Date Entry Form called for, issued by the UC Registrar's Office.
  • Return Four (4) copies of this form to

    Registrar's Office, Foreign Students Sections

    together with the following:
    • Three (2) Official copies of the Transcript of School Records.
    • Five (5) copies of the personal history statement property accomplished.
    • Five (5) copies of Affidavit of Financial Support, duly notarized, executed of your financial sponsor.
  • Application fee of .00 or its equivalent to cover the processing and mailing charges.
  • Admission Fee of P1,500.00 for each new Foreign Students
  • Service Fee of P1000.00 every Semester excluding Summer, and
  • Passed successfully Eye and Hearing Examination. (at UC-METC Clinic)
